Major Duties Include But Are Not Limited To:

• Provides nursing support to inpatient medicine and specialty teams, including assisting with discharges from outside hospitals. Additionally, the incumbent provides nursing support to PACT teams, specialty clinics, and community-based outpatient clinics.
• Supports and lead in the provision of appropriate care for Veterans ranging from 18 through the geriatric years.
• Evaluates care and outcomes to ensure timely and appropriate provision of services.
• Influences care outcomes by collaborating with members of the interdisciplinary team and executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex client care.
• Coordinates care with multiple providers across all levels and sites of care.
• Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice.

Pay: Competitive salary, regular salary increases, potential for performance awards

Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule: Full- Time, Tuesday - Saturday, 8:00am - 4:30pm
