Summary: The Program Manager oversees the daily operation of a Day Habilitation program for people with disabilities. Responsibilities include supervision of staff (hiring, discipline, and training of staff members), supervising staff activities, coordinating staff activities to meet the program objectives, and overseeing member services.

Schedule: Monday-Friday 8:00am-4:00pm (40 hours per week)

Essential Duties & Responsibilities:

1. Maintain appropriate certifications (i.e. First Aid, CPR, OSHA, Fire Safety, Human Rights, etc.) and ensure that all staff maintain their certifications.
2. Provide leadership to staff by creating a cohesive team environment.
3. Oversee the administering and interpreting of assessments to determine each individual's skill level on an annual basis.
4. Oversee and assure that all documentation is completed within timelines and meets Mass Health regulations.
5. Supervises and manages all data collection.
6. Assist Clinical Team and staff with referrals and development of services within the Day Habilitation site.
7. Maintain budget for the Day Habilitation program within allowed spending.
8. Maintain communication and collaborative relationships with other service providers and DDS.
9. Serve as liaison between Day Services, Residential and family for individuals.
10. Maintain sites according to DDS, Mass Health, CARF and Agency standards.
11. Ensure the staffing needs of the program are met. This includes coordination of work schedules, hiring of new staff, and coordinating the use of per-diem and agency relief staff, as needed.
12. Positively manage staff by conducting monthly supervisions with each staff and team meetings, supporting, coaching and mentoring staff to develop skills and abilities, and providing recognition opportunities for staff.
13. Provide timely Performance Evaluations. Ensure performance standards are met, and proactively address performance concerns, working with program management and Human Resources, as appropriate.
14. Ensure self and employees maintain certifications (such as CPR/1st Aid, MAP, PPS and PABC, as required) and complete all other training requirements of Bridgewell and the program's funding sources.
15. Work collaboratively with clinical and nursing teams to ensure the behavioral and medical needs of the people served are met. Collaborate with all service providers, ensuring communication and documentation is clear regarding the coordination and management of each person's needs.
16. Manage the technology needs of the program, including employee time and attendance, and data management/electronic health record systems. Ensure information is tracked as required, and system needs and issues are raised to the IT Department or Payroll Department, as appropriate.
17. Ensure the confidentiality of Personal Health Information, records of people served, and other related information is in compliance with HIPAA, and other state or federal mandates.

19. Other duties as assigned.

Required Education/Experience:

1. Bachelor's degree in a health-related field with a minimum of 2-years relevant healthcare experience and minimum of 1 year in a supervisory role within a relevant healthcare setting. OR
2. High School diploma with a minimum of 6 years of relevant healthcare experience with 2 of those years in a demonstrated leadership capacity or at least 1 year serving in a supervisor role.
3. Working experience in understanding the clinical needs of the people served is required.
4. A valid US driver's license and one year of driving experience is required.

Knowledge/Skills:

1. Knowledge of Day Habilitation regulations and CARF standards.
2. Knowledge of Behavior Management and Human Rights.
3. Excellent organizational skills, problem solving and communications skills.

6. Strong computer skills including the ability to coordinate data entry into a data management system, use Microsoft Office products proficiently, and records management systems.
7. Ability to be certified and maintain such certification, in PPS, PABC Tier 1 and 2, medication administration, First Aid/CPR and any other certification/training required based on program needs and Bridgewell policies and procedures.
8. Must have a current US driver's license and one year of driving experience.
9. Demonstrate ability to drive a 12-passenger van and/or a wheelchair van

Physical Demands:

The physical demands of staff working in residential programs assisting people with their care needs are varied and require the employee to be able to maneuver people of varying weights, and provide lifting, balance support and physical intervention in several scenarios.

Described below are essential requirements of staff to ensure the safety of employees and people served alike. While performing the duties of this job, the employee is required to be able to:

• Climb stairs, physically assist people served, often weighing more than 200 pounds, with movement, bathroom routines including toileting, assisting with entry and exit of a tub or shower, getting into and out of wheelchairs or dealing with assists that include physical, behavioral and medical interventions.
• Additional lifting or moving may be required as part of activities in the daily living of people in our services.
• Ability to drive a 12-passenger van

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time.
