GENERAL RESPONSIBILITIES:

The Director for Community Prevention and Treatment Services (CPTS) Program(s) is responsible for oversight, implementation, and continuous program improvements for HIV/AIDS, Hepatitis C primary medical care, medical case management, behavioral health integration, prevention, outreach and support services-designed to improve health outcomes for people living with or at-risk for HIV/AIDS, Hepatitis C, and/or other Sexually Transmitted Infections (STI). Central to this role is the development, implementation, and monitoring of processes, systems and protocols that support program planning, fiscal management, supervision, and evaluation of the activities related to HIV/AIDS, Hepatitis C (HCV), prevention and outreach programs.   

SPECIFIC RESPONSIBILITIES:

  Leadership/Administrative Responsibilities

· Provides leadership for the daily program activities of the Rayn White HIV/AIDS, HCV, SCC, TGNC prevention and outreach programs-supporting the principles of the Patient Centered Medical Home model of care for all CPTS patients. 

· Provides oversight and facilitates the multidisciplinary case conference consisting of multidisciplinary team members including physicians, nurse practitioners, physician assistants, nurses, medical case managers, behavioral health clinicians and SUD counselors, outreach staff, and peer support providers. 

· Fosters program culture centered on patient engagement, innovation, and compassionate care. 

· Implements and monitors activities necessary to ensure the integration of behavioral health services into HIV primary care through the use of collaborative care models and interdisciplinary treatment/service planning. 

· Serves as member of the Integrated BH Quality Management (CQM) committee.

· Collaborates with primary care and behavioral health leadership to ensure coordinated, integrated treatment/service planning

· Develops and supports an environment that is conducive to providing growth and development for employees.

· Monitors policies and procedures for HIV/AIDS/Hepatitis C services; ensuring program practices meet regulatory standards for care while remaining in alignment with the goals and objectives of the organization.

· Works in collaboration with colleagues within the finance department and other areas across the organization to ensure expenditure(s) reflect program workplans and remain in compliance with grant requirements; while maximizing available resources to improve patient outcomes. 

· Serves to represent Community Prevention and Treatment Services programs at community meetings, local, state-wide HIV/AIDS, Hepatitis C initiatives. 

· Maintains collaborative relationships with local, state partners, governing and accreditation bodies. 

· Participates in the writing of grants related to maintaining existing programs as well as obtaining additional funding opportunities. 

· Provides supervision, and mentorship to staff and interns.

· Investigates problems and suggest solutions to the community regarding public health issues, presented by the patients who are utilizing CPTS services. 

· Facilitates and leads regular staff meetings. 

Program Management

· Maintains screening, intake, admission, and treatment processes that promotes access to equitable care and services-to meet the needs of existing patients as well as new patients seeking care for HIV/AIDS, HCV, SCC, TGNC across the WYH network of care.  

· Ensures that the operation and processes for HIV/HCV counseling and testing are adequate to meet the needs of patients and all funding sources.

· Collaborates with the Sr. Director of Behavioral Health Services Lines to create seamless workflows- ensuring linkage(s) to Collaborative Care and BH Clinic services for CPTS patients in need of mental health, addiction, and/or co-occurring treatment services.

· Facilitates the development and maintenance of an effective Client Advisory Board.

· Regular observation and review of grant funded staff activities, assuring the effectiveness and fidelity of the implementation of interventions outlined in grant work plans.

Program Development and Grants Management

· Works in collaboration with the Sr. Director and CBHO in the conduct of market analyses relative to HIV/AIDS/HCV funding based on trends in the HIV/AIDS/HCV community with consideration for State and Federal initiatives.

· Monitors opportunities for additional funding to develop new programming, enhance and expand existing programs.

· Serves as the Principal Investigator/Project Director for major grants awarded to WYH.

· Monitors and evaluates progress toward goals and objectives as described in work plans for funded grants that support the HIV/HCV/AIDS, prevention and outreach programs.

· Provides timely reports for both internal and external stakeholders. 

· Prepares and submits required program reports to state and federal funders for CPTS programs.

Fiscal Responsibilities

· Assists in the preparation of annual budgets for HIV/HCV, outreach, prevention grants and ensures appropriate spending patterns. 

· Collaborates with colleagues in finance to ensure the monitoring and processing of time and effort attendance records.

· Educates and mentors staff regarding strategies and best practices to fulfill budget expectations including cost containment, while maximizing productivity and revenue generation. 

Performance Improvement/Patient Safety Activities and Risk Management

· Integrate and articulate institutional performance improvement activities into CPTS program services and initiatives.  

· Conduct chart/documentation reviews, quality improvement and program evaluation activities in accordance with program requirements.

· Conduct performance improvement activities reflective of program workplans and organizational CQI projects.  

Miscellaneous

· Demonstrates excellence in both internal and external customer service.

· Understands and effectively communicates HIPAA compliance, corporate compliance and client confidentiality.

· Ensures and/or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA-PCMH).

· Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center.

· Completes other duties as assigned.

Requirements
MINIMUM QUALIFICATIONS:

Graduate degree in Social Work, Counseling, Health Administration, Public Health or an allied health field. At least five (5) years of professional experience in a health-related facility. A minimum of three (3) years functioning in a supervisory/management capacity. Comprehensive knowledge of grant writing techniques, budgeting and program management. Prior experience using an electronic health record. Demonstrate an understanding of the HIV and HCV disease treatment and prevention strategies. Must demonstrate excellence in oral and written communication skills and analytical ability.  Possesses an understanding of the impact of the social determinants of health on patient engagement and outcomes. 

PREFERRED QUALIFICATIONS:

Preference for a licensed clinician in a related clinical discipline. With (3) three years’ of management experience in a healthcare setting, with HIV/HCV Services. Experience working with mental health, substance use disorders. Grant writing skills and budget experience.

Salary range: $80,000 - $100,000 annually
