Director of Rehab | Therapy Program Manager | Full-Time (Johnston, Iowa)

The ideal candidate may come from a Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) background, though Physical Therapists (PT), Occupational Therapists (OT), and Speech-Language Pathologists (SLP) are also encouraged to apply.

About the Role

Concept Rehab is seeking a Director of Rehab (DOR) to lead our skilled nursing and post-acute rehabilitation program in Johnston, IA. The ideal candidate may come from a Physical Therapist Assistant (PTA) or Certified Occupational Therapy Assistant (COTA) background, though Physical Therapists (PT), Occupational Therapists (OT), and Speech-Language Pathologists (SLP) are also encouraged to apply.

This is a full-time leadership position responsible for clinical oversight, team leadership, and program performance within a collaborative, mission-driven CCRC environment.

Compensation is based on experience, discipline background (PT, OT, SLP, PTA, or COTA), and leadership scope. Final compensation will reflect clinical experience, prior leadership experience, and market factors.

What You’ll Do

Lead and manage the rehabilitation team (PT, OT, SLP, PTA, COTA)
Oversee day-to-day therapy operations in a skilled nursing setting
Ensure high-quality patient care and clinical outcomes
Maintain documentation accuracy, compliance, and regulatory standards
Monitor therapy utilization, productivity, and program performance
Collaborate with nursing, administration, and interdisciplinary teams
Support staff development, training, and retention

Requirements:

Qualifications

Licensed PT, PTA, OT, COTA, or SLP (Iowa license required)
Experience in skilled nursing facility (SNF) or post-acute rehab
Prior Director of Rehab / Rehab Manager / supervisory experience preferred
Strong leadership, communication, and organizational skills
