Patient Navigator - BILH Weight Management Program ( On-Site / Hybrid / Remote )

The Patient Navigator supports the care of our patients in accessing and maintaining continuity with the healthcare team and assists patients in achieving their health‑care goals. Based on current practice workflows and strategies, he/she will develop practice improvement initiatives for ongoing quality improvement. Primary Responsibilities Perform direct outreach to patients to initiate patient engagement and enrollment with the clinic, schedule the initial appointment with a clinician, and provide follow‑up assistance as needed. Proactively contact patients to resolve potential barriers for appointments or program enrollment, reviewing patient records for enrollment requirements, and maintain and review KPI dashboards with program leadership on a recurring basis. Leverage Motivational Interviewing techniques or similar tools to engage patients and provide emotional support throughout the care journey. Document patient communication in the Electronic Medical Record (EMR) using encounter notes, basket messages, MyChart, and internal program management software. Verify and update patient insurance information when scheduling visits. Provide general clerical support, including filing, making appointments, photocopying, faxing, preparing and sending mail, making reminder phone calls, and maintaining the contacts database. Answer incoming calls to the clinic and elevate any patient issues to the appropriate team member. Contribute to workflow and process improvements and develop new ideas that impact the program. Required Qualifications Education: High School Degree or GED; college level study is a plus. Experience: No prior experience is necessary; prior customer service or patient care experience is a plus. Strong communication and interpersonal skills; ability to work independently and in a team. Ability to think creatively and recommend solutions to problems. Experience building relationships with patients and care team members in a healthcare or community‑based setting (preferred). Prior customer service experience (preferred). Completed Motivational Interviewing (MI) training (preferred). Ability to work in a multicultural diverse environment as a member of a healthcare team. Strong oral and written communication, interpersonal, organizational, and record‑keeping skills. Ability to manage multiple tasks and responsibilities simultaneously; work independently and as part of a team. Ability to maintain confidentiality and sensitivity to cultural differences, and to understand basic medical terminology. Ability to be flexible and easily adapt to change. Familiarity with Microsoft Office applications including Outlook, Teams, SharePoint and Excel. Pay Range $26.00 – $34.99 per hour. The listed range represents the base hourly wage the organization reasonably expects to pay for this position at this time. Actual compensation is determined by factors such as seniority, education, training, relevant experience, certifications, geography, job responsibilities and other permissible factors. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay and other additional pay practices in accordance with applicable law.
