Physical Therapist (PT) – Home Health | Avenel, NJ | PRN & Part-Time | $65–$85 Per Visit Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one care to patients in their homes in and around Avenel, NJ. Choose between part-time W-2 employment with benefits or a flexible PRN opportunity based on your availability. This opportunity is ideal for a licensed Physical Therapist looking to build a steady part-time caseload or add flexible PRN visits to an existing schedule without the double-booking, productivity quotas, and clinic burnout common in traditional outpatient settings. Treat one patient at a time, manage your availability, and provide individualized care with support from an established clinical and administrative team. What You'll Do: As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout the Avenel area. Responsibilities include: Perform physical therapy evaluations and develop individualized plans of care. Provide one-on-one treatments based on each patient's functional needs and goals. Manage individualized treatment plans with clinical autonomy. Help patients improve mobility, strength, balance, safety, and overall function. Complete, accurate, and timely clinical documentation. Communicate with patients, families, physicians, and members of the clinical team. Coordinate with our administrative team regarding scheduling, patient care, and logistics. Build and manage your caseload based on your availability and preferred coverage area. Compensation: Part-Time W-2:

• 65 per follow-up visit.
• 75 per 4-unit evaluation.

PRN:

• 72 per 4-unit follow-up visit.
• 85 per evaluation.
