LPN (Licensed Practical Nurse)

Job Summary

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our team in providing high-quality patient care within a Long-Term Care/Skilled Nursing Facility. The LPN will be responsible for delivering nursing care to residents, collaborating with healthcare teams, and promoting patient well-being.

Job Details

Unit Type: LTC/SNF
Shift Length: 12 hours
Guaranteed Hours: 36 hours per week
Years of Experience Required: 2 years
Required Certifications: BLS Certification
Weekend Requirement: Every other weekend
Holidays Requirement: As needed

Responsibilities

Provide nursing care under the supervision of registered nurses (RNs) and physicians.
Administer medications and perform treatments as prescribed.
Monitor patients' health status and report changes to the healthcare team.
Assist in developing and implementing patient care plans.
Maintain accurate documentation of patient records using EMR (Point Click Care).
Collaborate with interdisciplinary team members to provide comprehensive care.

Patient Population

The LPN will work primarily with geriatric patients in a long-term care setting, addressing their unique healthcare needs and promoting their dignity and quality of life.

Orientation and Training

New hires will participate in location-specific orientation, which includes a briefing one hour before the shift and possible shadowing of experienced staff to ensure a smooth transition into the role.

Additional Notes

The nurse-to-patient ratio is 1:15.
Shifts may vary and include both 8-hour and 12-hour options.
Orientation involves coming early and shadowing another team member when possible.
