Social Worker (Program Coordinator) Hematology/Oncology Clinic

Overview This Social Worker serves at the VA Eastern Colorado Health Care | Veterans Affairs. The incumbent is part of an interdisciplinary team that provides high-quality case management, clinical treatment, advocacy, and coordination of care to Veterans with complex health and mental health needs. Qualifications U.S. citizen or eligible non‑citizens if no qualified citizen available. Master’s degree in social work from a fully accredited school of social work (per CSWE). A doctoral degree cannot substitute. State licensure or certification to practice social work at the master's level. Minimum of one year of experience equivalent to GS‑11, demonstrating advanced practice skills and progressive professional competency. Experience in medical or oncology social work, transplant evaluations, end‑of‑life care, group therapy, or hospice/palliative care is preferred. Evidence of the ability to conduct psychosocial assessments, develop treatment plans, and collaborate with multidisciplinary teams. Demonstrated knowledge of program coordination, policy development, staff supervision, and training. Competence in maintaining patient privacy and confidentiality, and in advocating for community resources. For current VHA employees in this occupation, coverage of the Grandfathering Provision may apply. Responsibilities Assess patients’ understanding of diagnoses and treatment options. Conduct psychosocial assessments using tools such as the National Comprehensive Cancer Network distress thermometer for oncology patients. Perform social work transplant evaluations for bone marrow/stem cell candidates. Provide counseling and education to Veterans and families on coping strategies, self‑relaxation, and life adjustments. Facilitate oncology support groups and cancer‑specific educational programs. Collaborate with inter‑ and intra‑agency partners to coordinate care and resources (e.g., transportation, referrals). Assist Veterans with financial, familial, and other concerns related to treatment decisions. Advocate for hospice, home health, and adaptive equipment when appropriate. Develop and document end‑of‑life plans, advanced directives, and transition to palliative care. Coordinate discharge planning and placement into residential or skilled nursing facilities. Provide coverage for program operations and supervision of multidisciplinary staff. Physical Requirements Work is sedentary; light exertion required for walking, standing, bending, or climbing stairs. No special physical demands beyond routine duties. Benefits / Work Schedule Full‑time, Monday to Friday, 7:30 am‑4:00 pm. Paid time off: 37–50 days per year (including annual leave, sick leave, and Federal holidays). Parental leave: up to 12 weeks paid after 12 months of employment. Childcare subsidy for eligible employees after 60 days of employment. Retirement: Federal pension (5 years vesting) and 401(k) with up to 5 % VA contribution. Health, vision, dental, term life, and long‑term care insurance programs available.
