Job Summary

Aviata Health Group is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Aviata at the Gardens - Tallahassee, a skilled nursing and rehabilitation center in Tallahassee, Florida. As a LPN, you will play a vital role in providing patient-centered care to our residents and patients, while advancing your skills in a dynamic and supportive environment.

Job Responsibilities

As a Licensed Practical Nurse (LPN), you will:

• Provide direct patient care to residents and patients under the direction and supervision of a professional nurse

• Assist in the implementation of individualized treatment plans for each assigned resident

• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation

• Oversee Falls Program and Pressure Sore Program

• Provide nursing services, including conducting and documenting thorough evaluations of each resident's medical status

• Perform other duties as assigned

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN

• Experience in Skilled Nursing/Rehabilitation facilities preferred

• You must be qualified, compassionate, and dedicated to a job well done
