Job Description
Home Care Coordinator - Registered Nurse
Pay Range: $90,000–$102,000
Responsibilities:
- Lead assessments, care planning, and skilled nursing for home care clients
- Build relationships with clients, families, and physicians
- Mentor and empower direct care staff, ensuring compliance with regulations
Qualifications:
- Clinical nursing expertise and home care leadership experience
- Celebration of aging and ability to build trust
- Strong organizational skills and regulatory knowledge
