Duties/Responsibilities: Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsformember Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers Navigatemanagedcareplansforcommunityservicesandprograms Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicessupport Accurateandtimelydocumentationofpatientencountersandsessionsinallclinicalmanagementsystems Proactivelyidentify methods to improve ArchWellHealth's approach based onfeedbackandregularlyconductedsurveys Support advance care transitions with members and their families Assist members with access to state-based prescription programs and other benefits RequiredSkills/Abilities: Experiencesupportingpatientswithneedforsocialservices Experiencewithscreening,assessment,andplanningforcommonsocialservicesneeds Workingknowledgeofdifferentialdiagnosisofcommonmentalhealthconditions Stronginterpersonalcommunicationskillswithexceptionalactivelisteningabilities Highlyempathetic,non-judgmental,andopen-minded Experienceinacollaborativeteamenvironment EducationandExperience: Fully licensed LMSW in the desired State of practice Master'sdegreeinSocialWork 1+years'experienceinclinicalsocialwork Aproblem-solvingorientationandaflexibleandpositiveattitude Missiondrivenandmotivatedtojoinanorganizationthatwilltransformthewaywedeliveraccessible,clinicallyexcellentcaretoseniors ProficientPCskills FluencyinSpanishorotherlanguagesspokenbypeopleinthecommunitiesweserve(wherenecessary) ArchWell Health is an equal opportunity employer.
