Grafton Oaks Rehabilitation and Nursing Center is seeking an experienced and organized Social Services Director to lead our social services department. We are a 100% in-house facility under new management, and we are focused on strong customer service, smooth admissions, and supportive interdisciplinary teamwork.

Responsibilities:
• Serve as the primary contact for residents and families regarding care planning and support
• Manage the admission and discharge process, including documentation, assessments, and care coordination
• Work closely with nursing, therapy, and administration to ensure quality outcomes
• Maintain compliance with state and federal regulations
• Participate in resident care conferences and provide psychosocial support
• Oversee and mentor the Social Services team
• Assist with insurance authorizations, community resources, and discharge planning
• Maintain accurate records, progress notes, and care plans

Qualifications:
• Social Services Designee (SSD), LSW, or related certification required
• Experience in long-term care required
• Strong knowledge of Medicare/Medicaid, discharge planning, and psychosocial needs
• Excellent communication, time management, and documentation skills
• Ability to work professionally with residents, families, and staff
