What You'll Do

As an Occupational Therapist with Elara Caring, you will:

• Evaluate patients to determine rehabilitation needs and develop individualized plans of care

• Perform initial evaluations and ongoing reassessments according to OASIS and PPS regulatory guidelines

• Implement and adjust treatment plans to ensure quality outcomes and continuity of care

• Communicate patient progress, significant changes, or concerns to the physician and care team

• Educate patients and caregivers on activities of daily living (ADLs) , adaptive equipment, and home safety

• Assist patients and families in obtaining necessary equipment, including custom splint fabrication when appropriate

• Provide instruction and support for home therapy programs

• Collaborate with interdisciplinary teams to ensure the highest quality patient care

• Supervise and provide clinical guidance to Certified Occupational Therapy Assistants (COTA), Home Health Aides (HHA), and other clinical staff

• Ensure documentation and care delivery meet professional standards and regulatory requirements

This role involves working in a physically active and fast-paced environment , including patient transfers, lifting, and travel between patient homes.

What You'll Need

• Graduate of an accredited Occupational Therapy program

• Current, unrestricted Occupational Therapist license in the state of practice

• Minimum 1 year of experience in a clinical care setting

• Ability and willingness to travel within the assigned coverage area (approximately 50%)

• Valid driver's license, reliable transportation, and current auto insurance

• Ability to lift, push, pull, and support up to 50 pounds , including patient transfers and equipment movement

Reporting Structure

This position reports to the Branch Director, Clinical Manager, Clinical Supervisor, or designee .
