We are currently searching for our newest Population Health Pharmacy Technician who is passionate about providing outstanding customer service to our community. Position Details: Location: Remote - Must reside in the state of FL Status: PRN, hourly Shift Hours: 8:00AM - 4:30PM Weekend Work: None On Call: No Population Health Pharmacy Technician, under the immediate oversight of the Pharmacist, responsibilities include: Support the mission of the hospital and pharmacy department Notify the pharmacy/provider of missing prescriptions Request medication refills Assists the PHSO with medication-related quality measures for the PHSO. Contact pharmacies to confirm prescriptions were received and picked up Call patients to remind them to pick up their medications Obtains medication lists and engages in discussions with patients about medication adherence to identify and resolve patient adherence barriers that do not require clinical intervention. Request records from clinics and facilities as needed Will involve data analysis, e.g., payer data, claims, etc., research, and problem-solving to obtain resolution. Send referrals to other disciplines to coordinate care Required Qualifications: High School Graduate or Equivalent GED Required FL Registered Pharmacy Technician License Preferred Qualifications: 1 year Pharmacy Technician experience Pharmacy Technician Certification (CPhT) PTCB certification required within 6 months of hire.
