Day shift, Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).

Job Description

We are seeking a dedicated and compassionate Physical Therapist (PT) for a home health position. This role involves working with patients in their homes to improve mobility, reduce pain, and enhance the quality of life through tailored therapy interventions. The position requires a Missouri PT state license, with compact privileges possible.

Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Implement therapy interventions to improve patient mobility and quality of life.
• Monitor progress and adjust treatment plans as necessary based on patient needs.
• Document patient care and progress in the electronic medical records (EMR) system (Meditech).
• Maintain a caseload of 8-11 patients daily, depending on travel time.

Required Experience / Certifications / Licensure

• At least 1 year of experience as a Physical Therapist.
• Missouri PT state license (PT compact privileges possible).
• Basic Life Support (BLS) certification required.
