Clinical Dietitian - Outpatient and Home Infusion

Job Overview Clinical Coordinator for Outpatient and Home Infusion Pharmacy. Responsible for coordinating and educating referral sources and patients, ensuring new patient referrals transition into the home infusion setting. Provides clinical expertise to assist referral sources and/or patients with identification of appropriate candidates for home infusion, assessment of patient or caregiver ability to manage care, patient education on therapy and supplies, and clarifying expectations of home care. Key Responsibilities Update and educate referral sources on Pharmacy services. Coordinate clinical care from referral source into the home setting. Assist referral sources and patients with identification of appropriate candidates for home infusion. Assess patients’ or caregivers’ ability to manage care at home. Educate patients and caregivers on therapy and supplies. Clarify expectations of care in the home and coordinate medication connection at the hospital or facility to expedite discharge. Complete nutritional assessments, prepare nutrition care plans, recommend nutritional therapies, and monitor effectiveness of nutritional care. Provide nutrition education to promote improved health and ensure compliance with regulatory requirements. Conduct and participate in community education, marketing, and public relations efforts. Participate in in‑services and continuing education programs. Collaborate with referral sources to facilitate coordination of care for new patient referrals. Coordinate and communicate all discharge planning activities for new patient referrals to appropriate personnel and sources. Assess patients’ nutritional needs and create nutrition care plans. Provide nutritional education and counseling. Provide education for other home infusion therapies as appropriate. Document records completely and accurately. Support departmental goals. Adapt behavior to the specific patient population, respecting privacy and communication style. Perform additional related duties as required. Qualifications Education: Bachelor of Science degree in Nutrition, Dietetics or related field. Required Work Experience: 1 year clinical experience in nutrition, with knowledge of normal and therapeutic nutrition, nutrition screening, support principles, and modified diets. Preferred Work Experience: 1 year home infusion experience. Certifications: Registered Dietitian status with the Commission on Dietetic Registration (CDR) and current licensed Dietitian/Nutritionist status with LBEDN. Knowledge, Skills, and Abilities (KSAs): Computer skills and dexterity for data entry and information retrieval. Effective verbal and written communication skills, capable of presenting information clearly and professionally. Proficiency with Windows‑style applications and related software. Strong interpersonal skills. Commitment to professional development and evidence‑based clinical services. Excellent analytical skills to assess nutritional status and develop appropriate care plans. Excellent organizational and prioritization skills to meet frequent deadlines. Physical and Environmental Demands Frequent exertion of 10–20 pounds of force to move objects; occasional exertion up to 100 pounds. Physical demands exceed those for sedentary work.
