Physical Therapist

$2720.00/week - Job Summary: Provides comprehensive physical therapy services to patients, focusing on restoring and improving physical functions through individualized treatment plans. Collaborates with interdisciplinary teams to enhance patient care and outcomes. Responsibilities: Conducts detailed evaluations to assess patient needs and establish customized treatment goals. Implements evidence-based therapy interventions using appropriate clinical techniques and protocols. Develops and communicates individualized care and discharge plans to patients and team members. Supervises assistant staff and ensures clinical documentation is accurate and completed within deadlines. Performs ongoing assessments to monitor progress and adjust treatment strategies accordingly. Ensures compliance with all licensure, certification, and organizational policies. Qualifications: Holds a Doctor of Physical Therapy (DPT) degree, obtained or licensed after January 2016, or equivalent foreign credential. Maintains current physical therapy licensure in all applicable states of practice. Possesses valid CPR/BLS certification from an approved provider. Demonstrates at least two years of recent experience in the field, including home health, with proficiency in HCHB, OASIS, and EPIC. Is at least 21 years old with a valid driver’s license and clean driving record. Complies with vaccination requirements and participates in E-Verify.
