Job Summary As the Masters Social Worker at St. Anthony Hospital, you will perform social work assessments and interventions for hospitalized and emergency department patients, including crisis intervention, patient/family counseling, high‑risk screening, referrals for financial or other resource needs, arranging and facilitating family/patient representative meetings with the health care team, arranging post‑acute placement on complex discharges, and engaging appropriate agencies or community resources for high‑risk patients.
