About the Position

Job Title: PACU RN Profession: RN (Registered Nurse) Specialty: PACU (Post Anesthesia Care Unit) Duration: 13 weeks Shift: 12H Days Hours per Shift: 34.5 hours per week Experience: Minimum of 2 years License: Required Certifications: BLS Certification, BLS (AHA) Must-Have: - FLU VACCINE REQUIRED - NO EXEMPTIONS OR EXCEPTIONS - ICU background and acuity experience - Pediatric experience Description: Candidates must have experience with higher acuity patients in PACU as boarders. Patients may wait longer for beds than their recovery time as boarders. Strong indicators of the ideal candidate include background in SICU, Transplant ICU, Neuro ICU, or mixed-acuity ICU. Candidates should manage vents, vasoactive drips, and invasive monitoring. Comfort with rapid deterioration and escalation is a must. Candidates with only step-down or telemetry experience will not be considered. Candidates must be able to clearly describe acuity or critical decision-making. Experience transitioning from ICU to PACU is preferred. Candidates should be familiar with rapid assessment, anesthesia recovery, and airway vigilance. Understanding the differences between short-term, high-risk monitoring versus long-term ICU management is essential. Prior PACU exposure is necessary. Candidates should not think of PACU as less acute. A limited understanding of anesthesia-related complications is not acceptable. Experience caring for kidney transplant patients post-operatively is required. Candidates should discuss strict I&O, urine output trends, and relevant labs such as Cr and K+. Awareness of immunosuppression risks is crucial. Ability to identify early signs of rejection, bleeding, or delayed graft function is necessary. Candidates should be prepared to express concerns in the first few hours after a kidney transplant, including urine output, hemodynamics, electrolyte shifts, and graft perfusion. Experience recovering patients after Neuro IR procedures, such as thrombectomy or aneurysm coiling, is preferred. Familiarity with frequent neuro checks is expected. Candidates should control BP parameters tightly and be aware of access site monitoring (femoral/radial). Immediate escalation of care post Neuro IR in response to acute neuro change, decreased LOC, new weakness, severe headache, or access site bleeding/hematoma is required. Experience caring for bariatric patients in the post-op setting is necessary. Candidates should discuss airway risk, OSA, hypoventilation, and safe mobilization equipment awareness. Pain control must be mindful of respiratory depression. Candidates should express concerns about airway obstruction, rapid desaturation, CO2 retention, or aspiration risk in bariatric patients. Candidates should describe a situation where they managed or escalated an airway concern. Early recognition and calm escalation with collaboration with anesthesia are paramount. Familiarity with CPAP/BiPAP or reintubation preparation is expected. Candidates must demonstrate clear thinking and ownership of decisions in high-stress situations. Effective communication with anesthesia and surgeons for concerns is essential. This position requires strong prioritization skills in managing multiple high-acuity patients simultaneously. Candidates should use protocols and time-based reassessments and ensure clear communication with anesthesia and surgeons during care. Reflective learning after stressful events is also important.
