We are looking for a compassionate and organized Case Manager to support formerly homeless adults in maintaining stable housing and improving overall well-being in San Francisco, California. This Long-term Contract position focuses on delivering tenant-centered case management, coordinating access to community resources, and fostering a safe, supportive residential environment. The ideal candidate is skilled at balancing documentation, crisis response, and relationship-building while working with individuals facing behavioral health, substance use, and medical challenges.

Responsibilities:
• Oversee a high-volume portfolio of residents and provide consistent case management tailored to each individual’s housing stability and personal goals.
• Meet with tenants regularly through outreach, intake conversations, and ongoing check-ins to evaluate needs, monitor progress, and adjust support plans as appropriate.
• Help residents secure and maintain public benefits, stay current with rent obligations, and address barriers that may affect continued tenancy.
• Work alongside property and hotel staff during unit reviews, pest control visits, and habitability follow-up to support safe living conditions.
• Connect tenants with healthcare, behavioral health, employment, and community-based services, and encourage successful engagement with referred programs.
• Strengthen resident participation by organizing group activities, community gatherings, and other events that promote connection and mutual support.
• Respond to urgent tenant situations with calm de-escalation, sound judgment, and timely coordination with internal teams or external providers when needed.
• Keep accurate, confidential case records, including assessments, service notes, and file maintenance in accordance with program expectations.
• Contribute to recurring program-wide activities such as income reviews, benefit renewals, and resident support during scheduled check distribution days.
• Partner with colleagues and service providers to advocate for tenants, address concerns promptly, and report suspected abuse or neglect through proper channels.• Experience providing administrative or case management support in housing, social services, behavioral health, or a related setting.
• Ability to manage a large caseload while maintaining organized records, timely follow-up, and attention to detail.
• Strong communication and interpersonal skills, with the ability to build trust with diverse tenant populations.
• Comfort working with individuals affected by mental health conditions, substance use disorders, or complex medical needs.
• Proficiency in administrative tasks such as data entry, file organization, and maintaining accurate documentation.
• Ability to respond effectively to crisis situations, use de-escalation techniques, and exercise sound judgment.
• Capacity to collaborate with multidisciplinary teams, property staff, and outside community partners.
