Alzheimer’s Case Manager - Center of Excellence

Albany Medical College seeks a Case Manager to join the Center of Excellence for Alzheimer’s Disease within the Community Neurology division. You will perform social work activities, assess patients, and provide ongoing support to patients and families, reporting to the Neurology department.
