Supervise and direct activities of nursing staff at multiple levels; coordinate care with other disciplines while applying critical thinking and independent judgment. Document care plans, monitor patient status, and communicate changes to the interdisciplinary team; ensure continuity of care. Administer medications and treatments as ordered; monitor for adverse reactions and respond to changes in patient condition. Educate patients and caregivers; provide support and guidance to families within the home hospice context; participate in discharge planning when appropriate. Adhere to infection control, safety, and quality standards; maintain accurate charting and regulatory compliance.
