Licensed Clinical Social Worker (LCSW) - Brooklyn, NY

Job Type: Full-Time (On-site)

Position Summary:

We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our behavioral health team. This position provides direct clinical services to adolescent and adult patients in primary care and school-based clinic settings.

The LCSW will provide high-quality, patient-centered behavioral health services through individual, family, and group therapy while ensuring compliance with clinical documentation standards and mental health care requirements. This role works collaboratively with an interdisciplinary healthcare team to support comprehensive patient care.

**Fluency in English and Spanish is a plus**

Key Responsibilities:

• Collaborate with behavioral health leadership and clinical teams to support mental health service goals for adolescent and adult patients.

• Conduct comprehensive psychosocial assessments and identify patient needs.

• Develop individualized treatment plans and coordinate appropriate referrals, including psychiatric evaluations, hospitalization, and community-based services.

• Partner with healthcare staff to implement behavioral health screenings for depression, anxiety, substance use, alcohol, and tobacco.

• Provide individual, family, and group psychotherapy based on patient assessments and treatment goals.

• Coordinate therapy sessions and support patients and families throughout treatment.

• Provide crisis counseling and intervention services as needed.

• Communicate and coordinate care with case managers, community agencies, and other care partners.

• Complete psychosocial evaluations and regularly update treatment plans.

• Maintain accurate and timely documentation in the electronic health record (EHR).

Required Qualifications:

• Master's degree in Social Work from an accredited program.

• Active and unrestricted New York State LCSW license.

• Minimum of 5 years of social work experience.

• Bilingual English/Spanish preferred.

• Completion of Mandated Reporter Training.

• Current CPR and Infection Control certifications.
