Home Care Field Registered Nurse (RN) - Washington County, NY | $85K Location: Washington County, New York Company: At Home Care Partners Salary: $85,000 per year Position: Full-Time Field Registered Nurse (RN) Bring Compassionate Nursing Care Directly to Patients At Home Care Partners is seeking a skilled and compassionate Home Care Field Registered Nurse (RN) to join our clinical team serving patients throughout Washington County, New York . This full-time position is an excellent opportunity for an RN who enjoys working independently, connecting one-on-one with patients and families, and providing clinical support in home and community-based settings. As a Field RN , you'll travel throughout Washington County to assess patients, coordinate individualized care, supervise home health aides, and help ensure each patient receives safe, appropriate, and high-quality services. What You'll Do Travel to patient homes and community-based settings throughout Washington County Conduct comprehensive in-home nursing assessments Perform head-to-toe evaluations, including medication and diagnosis reviews Develop, review, and update individualized Plans of Care (POC) Complete required home health aide supervisions Provide clinical guidance and support to aides and caregivers Orient aides to each patient's Plan of Care and individual care needs Monitor patient conditions and promptly communicate significant changes or concerns Collaborate with patients, families, caregivers, aides, and office staff Complete accurate and timely clinical documentation Help maintain continuity, safety, and quality of patient care Maintain compliance with applicable healthcare regulations and company policies What We're Looking For Current and active New York State Registered Nurse (RN) license Ability and willingness to travel throughout Washington County and assigned service areas Medical Professional Liability insurance Strong clinical assessment and critical-thinking skills Excellent written and verbal communication abilities Strong organizational and clinical documentation skills Comfortable using Gmail and Microsoft Word/Excel Ability to work independently and effectively manage a field-based schedule Professional, dependable, compassionate, and patient-focused Must meet all company hiring and onboarding requirements Preferred Experience Home care or home health nursing experience Community health or outpatient nursing experience Conducting in-home patient assessments Completing aide supervisions Developing, reviewing, or updating Plans of Care Familiarity with Medicaid, Medicare, or MLTC plans Bilingual Spanish/English is a plus Compensation & Benefits Salary: $85,000 per year At Home Care Partners offers: Competitive compensation Weekly direct deposit Health insurance Dental insurance Vision insurance 401(k) plan Transit benefits and dependent care accounts Verizon Wireless and mobile internet discounts Company vehicle and parking access, where applicable Supportive team culture Opportunities for advancement Your Nursing Experience Can Make a Difference At Home Care Partners is a nonprofit 501(c)(3) home care agency committed to providing compassionate, individualized care to patients in their homes and communities.
