This is an excellent opportunity for an RN who enjoys working independently in the field, building relationships with patients and families, and providing personalized care in home and community-based settings. About the Opportunity As a Home Care Field RN , you will travel throughout Chenango County to complete nursing assessments, oversee patient care, support home health aides, and help ensure each client receives safe, individualized, and high-quality care. Our Field RNs are an essential part of our care team, serving as a clinical resource for patients, families, caregivers, and aides while helping maintain continuity and quality of care. Key Responsibilities Travel to client homes and community-based settings throughout Chenango County Conduct comprehensive in-home and community-based nursing assessments Perform head-to-toe evaluations, including medication and diagnosis reviews Complete aide supervisions and provide ongoing clinical guidance Develop, review, and update individualized Plans of Care (POC) Provide orientation and education to aides based on each client's Plan of Care Monitor client conditions and report significant changes as appropriate Communicate effectively with patients, families, caregivers, aides, and office staff Complete accurate and timely clinical documentation Maintain compliance with applicable healthcare regulations and company policies Help ensure services are delivered safely and according to each client's individual needs Requirements Current and active New York State RN license Ability and willingness to travel throughout Chenango County and assigned service areas Medical Professional Liability insurance Comfortable using Gmail and Microsoft Word/Excel Strong clinical documentation and communication skills Ability to work independently in a field-based environment Professional, dependable, compassionate, and organized Must meet all company hiring and onboarding requirements Preferred Qualifications Bilingual in Spanish and English Previous experience in home care, community health, or outpatient nursing Familiarity with Medicaid, Medicare, or MLTC plans Strong clinical assessment skills Experience developing, reviewing, or updating Plans of Care Comfortable working with patients from diverse backgrounds and with varying levels of care needs Compensation & Benefits Salary: $85,000 per year At Home Care Partners offers a comprehensive benefits package, including: Competitive compensation Weekly direct deposit Health insurance Dental insurance Vision insurance 401(k) plan Transit benefits and dependent care accounts Verizon Wireless and mobile internet discounts Company vehicle and parking access, where applicable Supportive team culture Opportunities for advancement Why At Home Care Partners?
