Schedule/Availability Requirements:

1. April 25 @ 5p through April 28 @ 7a
2. May 23 @ 5p through May 27 @ 7a
3. June 06 @ 5p through June 09 @ 7a

Setting: Emergency Dept./Surgery

FTE: 1

Duties: Cover Urology call - get handoff from Urologists if appropriate

Required Procedures: General Urology

EMR: EPIC

Support Staff: Surgery nurses and techs. on call

Deal breakers (do not present this): Not willing to learn EPIC

Travel Preferences:

• Preferred Lodging: Candlewood Suites
• Air Travel: Local, preferred but not required

Credentialing Information:

• Timeframe: 3-4 weeks minimum
• Requirements: M.D. or D.O.; DEA; NE license; case logs from last 2 years

Submission Requirements

• Board Certified/Board Eligible (Urology) - REQUIRED, NOTE AT NAME CLEAR.
• Active NE license - HIGHLY PREFERRED
• Clean Malpractice - HIGHLY PREFERRED
• Availability of dates in the job description - REQUIRED, NOTE AT NAME CLEAR.
• Case logs are required - REQUIRED, INCLUDE AT PRESENTATION.
• Disclose if the provider has worked at any other CHI/Dignity facility - REQUIRED, NOTE AT NAME CLEAR.
• COVID VACCINATED - REQUIRED, NOTE AT NAME CLEAR.
