Care Manager

Overview Pro Care by Abound Health is seeking a Case Manager to join our team in Southfield, MI. You’ll lead client care coordination, support families and staff, and ensure high-quality service delivery for individuals receiving Michigan Waiver services. This is a hands-on, impactful role where you’ll drive outcomes and make a real difference in people’s lives. Responsibilities Manage assigned client cases, ensuring services are delivered accurately and on time Review and maintain all client and staff documentation to meet regulatory standards Conduct client and staff meetings to ensure compliance with care plans and policies Develop and track individualized goals, interventions, and outcomes Coordinate staffing schedules and ensure proper coverage for all services Serve as the main point of contact for clients, families, and community partners Resolve concerns, document incidents, and ensure quality care delivery across all cases Qualifications Experience in care coordination, case management, or related healthcare/social services field Strong organizational skills and ability to manage multiple priorities in a fast-paced environment Excellent communication and relationship-building skills Detail-oriented with strong critical thinking and problem-solving abilities Ability to work collaboratively with clients, families, and interdisciplinary teams Proficiency in Microsoft Office and documentation systems Education Master’s degree + 1 year experience working with individuals with disabilities, OR Bachelor’s degree + 2 years of relevant experience, OR Associate’s degree/60 credits + 4 years of experience, OR High School Diploma/GED + 5+ years of relevant experience Compensation & Benefits Base pay is determined by factors such as location, experience, skills, and education. In addition to base pay, we offer a comprehensive benefits package including medical, dental, and wellness benefits starting on your 90th day of employment.
