Case Associate

The Case Associate provides administrative and clerical support for care coordination, including managing client intake and referrals, maintaining files and billing data, responding to inquiries, and collaborating with internal and external stakeholders to ensure timely and accurate service delivery.

Job Responsibilities

• 
  
  Provide administrative support to care coordination process; prepare and maintain program data and billing; monitor and respond to client inquiries, concerns and complaints.
  
  



• 
  
  Responsible for intake, processing of client referrals, and preparation of client files. Ensure timely follow-up to messages/inquiries made to care coordination team. Serve in a clerical capacity to review and monitor files and related documentation. Complete assigned billing tasks including data entry and maintenance. Provide pre and post assessment activities
  
  



• 
  
  Work with external partners to resolve billing matters. Attend all trainings as required. Provide support to clients, care coordinator, supervisors and management staff.

  Work collaboratively with internal stakeholders and team members as needed.

Education and Experience Requirements:

• 
  
  H.S. Diploma or General Education Degree (GED)

  Associate's Degree (AA) 1-3 years in a related field of study
