In Adult Outpatient Services, Access Coordinators play a vital role in connecting clients to the care and resources they need by guiding them through referrals, screenings, scheduling, and intake services. Through compassionate communication, thoughtful coordination, and a commitment to service, you'll help ensure individuals can access support quickly and confidently.

Pay: Range starts at $18.75/hour (pay is calculated based on years of licensed working experience)

Schedule: Monday Friday | 8:00am 5:00pm

Job Summary

The Access Coordinator receives and coordinates all incoming referrals to Valley and manages eligibility and program screening, scheduling, and intake and registration documentation. As a community and client-facing role, the Access Coordinator ensures a high level of professionalism, coordination, and efficiency.

Essential Functions

• Manages incoming referrals through telephone calls, emails, and other contact types. Gathers required information from referral sources.
• Completes eligibility and program screening with referred clients; schedules intakes and follow-up appointments
• Verifies insurance information and discusses funding options with clients
• Coordinates with program staff to ensure completion of registration paperwork and any necessary documentation
• Tracks program capacity and intake availability; manages program waitlists as appropriate
• Coordinates care by connecting referred clients to alternative treatment options and community partners as appropriate
• Participates in community outreach, engagement, and education regarding Valley services and the referral and intake process
• Tracks key access information including no-show rates, hospitalizations and follow-up, court tracking/reporting, and overall program and client engagement levels
• Ensures that grant-related, state and local, and payer access requirements are met
• Follows agency documentation policies for all phases of treatment, including assessments, care plans, treatment transitions and discharge plans, and client care
• Collaborates with internal and external multidisciplinary teams regarding clinical aspects of treatment
• Adheres to ethical and legal standards set by licensing boards and state regulations
• Meets direct care expectations.
• Participates in ongoing professional development activities to meet licensure, certification, and/or other agency requirements

Requirements

Education

• High school diploma or equivalent

Experience

• None

Licenses/Certificates

• Case Manager certification (Can be post-hire)
• CPR certification (Post-hire)
• Valley de-escalation certification (Post-hire)
• Driving positions require a minimum age of 21 and a current driver's license

Preferred Qualifications

• Bachelor's degree in social work or related field
• Previous social services experience
