Job Overview - Behavioral Health Case Manager
Compensation: $23 - $28/hour (Dependent on Experience)
Location: Philadelphia, PA
Schedule: Part Time (In-Office)


TAG MedStaffing is hiring a Behavioral Health Case Manager in Philadelphia, PA for our client, supporting care coordination, discharge planning, and connection to outpatient behavioral health services. The Behavioral Health Case Manager will assist individuals accessing mental health services by assessing resource needs, coordinating referrals, developing aftercare plans, and conducting follow-up outreach. This temporary opportunity requires strong case management skills and experience supporting children and adults within behavioral health or human services.


Responsibilities as the Behavioral Health Case Manager:

• Case Management: Coordinate individualized resources and services based on client needs, preferences, and established care plans.
• Aftercare Planning: Develop discharge and aftercare plans and connect clients with appropriate outpatient mental health and community-based services.
• Referral Coordination: Manage referrals and collaborate with clients, families, providers, and community resources to support continuity of care.
• Client Follow-Up: Conduct follow-up outreach to monitor service connections, address barriers, and support continued engagement with care.
• Compliance Monitoring: Monitor client needs and safety concerns while ensuring case management activities follow applicable program and regulatory requirements.
• Documentation: Maintain accurate and timely client records, referral information, coordination activities, and case documentation.

Qualifications for the Behavioral Health Case Manager:

• Education: Bachelor's degree is required, with a degree in Psychology, Sociology, Social Work, or a related human services field preferred.
• Licensure: Valid driver's license and access to a reliable vehicle are required.
• Experience: 1+ year of experience working with children and adults in behavioral health, case management, social services, or a related human services setting is required.
• Industry Knowledge: Familiarity with behavioral health services, discharge planning, community resources, outpatient referrals, and care coordination is preferred.
• Technical Skills: Basic computer, electronic documentation, data entry, and case management system skills are required.
• Skills & Attributes: Strong communication, organization, documentation, interpersonal, problem-solving, advocacy, and client engagement skills are required.
