Part-Time HouseCalls NP — Flexible Home Visits

Optum Home & Community Care is seeking an in-home Advanced Practice Clinician (Nurse Practitioner) for a part-time role in Texas. You will conduct comprehensive annual in-home health assessments for Medicare Advantage and other plans, with no prescribing responsibilities, and coordinate care with primary care providers. The role offers flexibility within 8am-8pm, 24 hours per week, and no on-call, weekends, or holidays.
