Responsibilities

• Administer prescribed medications and treatments per physician orders
• Monitor residents’ conditions, perform routine assessments, and document findings
• Assist with activities of daily living and mobility; support safety and falls prevention
• Collaborate with the RN and care team to update care plans and communicate changes
• Maintain accurate documentation in the resident’s medical record and report concerns promptly
• Respond to emergencies and provide high-quality basic nursing care within scope of practice

Requirements

• Active LPN license in New Hampshire
• Current CPR/BLS certification
• Prior long-term care or skilled nursing experience preferred
• Ability to work night shifts (23:00–07:30) and adapt to a busy environment
• Strong communication, observation, and documentation skills
