Home Dialysis Nurse Educator & Case Manager

Fresenius Medical Care

Fresenius Medical Care is seeking an RN Home Therapies Educator and Case Manager to support patients with peritoneal dialysis and home hemodialysis training. The role emphasizes home visits, education, and coordination with the care team.

You will train patients and Home Partners, assess learning needs, and participate in quality improvement activities while maintaining clinically competent care under supervision.
