Position Summary

As a Community Outreach / Referral Relations Specialist , you will play a vital role in connecting individuals and families to the resources and services they need. You’ll be the bridge between our organization and the community, fostering relationships, building trust, and ensuring that our mission reaches those who need it most.

Key Responsibilities

• Build and maintain relationships with community partners, organizations, and service providers.
• Act as a liaison to connect individuals and families with appropriate resources and referrals.
• Develop and implement outreach strategies to increase awareness of Golden Age Foundation’s programs and services.
• Represent Golden Age Foundation at community events, meetings, and networking opportunities.
• Collaborate with internal teams to ensure seamless integration of services and support.
• Track and report outreach activities and referral outcomes to measure impact and effectiveness.

Required Skills and Qualifications

• 1 year of experience in community outreach, referral coordination, or a related field.
• Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse groups.
• Knowledge of community resources and services, particularly in areas such as mental health, housing, education, or employment.
• Self-motivated and organized, with the ability to manage multiple tasks and priorities.
• Passion for helping individuals and families achieve stability and independence.
• A collaborative mindset and commitment to Golden Age Foundation’s mission and values.
