Social Worker MSW - Full-time - Weekends

The Social Worker is responsible for conducting comprehensive bio-psychosocial assessments, providing patient education, coordinating and implementing discharge plans, and ensuring that patient and caregiver emotional, social, cultural, and spiritual needs are addressed in collaboration with the multidisciplinary team. Responsibilities Conduct patient assessment interviews, provide referrals, and coordinate discharge plans. Identify and provide helpful resources to patients and families. Maintain patient records and communicate critical information to the medical team. Maintain professional and technical knowledge and required certifications through continuing education and development. Utilize management resources. Participate in on‑call rotation. Adapt behavior to the specific patient population, respecting privacy and communication preferences. Qualifications Education Required: Master’s degree in Social Work (MSW) from a University accredited by the Council on Social Work Education (CSWE). Work Experience Required: None. Preferred: Two years experience in a medical setting. Certifications Required: Certified Social Worker (CSW) verified by LABSWE or Registered Social Worker (RSW) certificate. Preferred: Licensed Master Social Worker (LMSW) verified by LABSWE or scheduled to pass the LMSW exam within 6 months of employment. Knowledge, Skills, and Abilities Computer skills for data entry and retrieval of required job information. Proficiency with Windows applications and related software packages. Effective verbal and written communication skills with ability to present information clearly and professionally. Working knowledge of common disease entities, medical terminology, and acute and chronic care settings. Understanding of psychosocial and economic effects of illness and trauma on patients and families. Knowledge of health care systems and public programs such as Medicare, Medicaid, Medicare Advantage, and commercial insurance plans. Strong interpersonal skills to engage patients, families, multidisciplinary team members, and community personnel. Physical and Environmental Demands The position requires moving objects weighing 20 to 50 pounds occasionally and 10 to 25 pounds frequently. The role may involve sitting, standing, walking, and operating mechanical lifting devices. The work environment may expose employees to communicable diseases and hazardous medications.
