A-Line Staffing is now hiring a Hybrid Registered Nurse – Division of Developmental Disabilities - Case Management.

• Location: Phoenix AZ 85012| Hybrid – 90% Remote / 10% Onsite
• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Hours: 40 hours per week
• Pay Rate - $40.00 an hour paid weekly
• Weekends/Holidays: None

Key Responsibilities

• Complete member reports in the format required by DES DDD, documenting medical necessity for skilled services based on individualized member needs.
• Conduct in-person member assessments every 90 days and as needed.
• Travel within the assigned service area to complete required field-based assessments.
• Complete nursing referrals and coordinate appropriate services.
• Communicate with vendors and place vendor calls as necessary.
• Collaborate with members, families, caregivers, support systems, and healthcare providers.
• Participate in multidisciplinary planning meetings to identify and determine appropriate services in accordance with applicable policies, guidelines, and DDD/AHCCCS requirements.
• Maintain timely, accurate, and comprehensive clinical documentation.
• Complete assigned documentation within established timeframes.
• Participate in internal and external interprofessional collaboration.
• Attend mandatory meetings and complete required annual training.
• Assist with special projects and other duties as assigned.
• Educate members and families by translating complex clinical information into clear, understandable guidance.

Required Qualifications

• Active Arizona Registered Nurse (RN) license in Arizona.
• 4–5 years of recent RN experience in one or more of the following:
• Home health
• Hospice
• Long-term care
• Nursing case management
• At least 2 years of experience working with individuals with disabilities.
• Recent hands-on experience caring for individuals with medical feeding tubes, including gastrostomy (G-tubes).
• Valid Arizona driver's license.
