Assignment Details:

Requirements:
- Licensed Clinical Social Worker (LCSW) in California
- One year of acute care experience required; two years preferred
- Familiarity with MediCal and Medicare
- Cerner experience strongly preferred
Roles & Responsibilities:
- Provide clinical social work services within an acute care setting
- Conduct assessments and develop appropriate treatment plans
- Collaborate with interdisciplinary teams to ensure holistic patient care
- Advocate for patients and families to access necessary resources and support
- Document clinical interventions and patient progress consistently
- Participate in discharge planning and follow-up care coordination
