Key Responsibilities

• Conduct thorough evaluations including the review of medical history and detailed assessments to determine effective treatment plans.
• Communicate evaluation findings and treatment strategies to patients and their families, actively involving them in the goal-setting process.
• Implement individualized treatment protocols that are informed by evaluation findings and adapt them as necessary based on patient responses and ongoing assessment.
• Collaborate with other healthcare professionals to ensure comprehensive patient care, documenting any changes in patient conditions that may necessitate modifications to the treatment plan.
• Prepare accurate discharge planning and aftercare instructions, ensuring patients receive the necessary follow-up for continued recovery.
• Maintain meticulous documentation of patient care activities to comply with evolving healthcare regulations, emphasizing clarity in initial evaluations, daily encounters, progress summaries, and discharge evaluations.
• Demonstrate efficient time management by meeting productivity targets while maintaining an accurate schedule.
• Participate in marketing initiatives to promote our services within the community.

Patient Population / Client Focus

Our facility primarily serves individuals recovering from surgery, injuries, and chronic pain conditions, with a strong emphasis on geriatric rehabilitation. Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.

• A minimum of 2 years of experience as a Physical Therapist, preferably in a rehabilitation setting.

Compensation and Benefits
