Our Lee Physician Group (LPG) Community Care Outreach team is growing and seeking a compassionate, collaborative, and resourceful Master's Level Social Worker (MSW) to join our mission-driven team.

Hybrid work environment Monday-Friday schedule (8 AM - 4 PM) No weekends or holidays Opportunity to make a meaningful impact in the community Collaborative and supportive team culture Work alongside healthcare professionals dedicated to improving patient outcomes

You'll be joining an incredible multidisciplinary team consisting of 6 Master's Level Social Workers and 5 Nurse Navigators. Together, we work to connect patients with resources, remove barriers to care, and improve overall health outcomes throughout our community.

Serve as a key resource for patients with social, financial, and healthcare-related challenges Visit 45 primary care clinics throughout the community approximately 12 days per week Collaborate with providers, nurse navigators, care teams, and community partners Assess patient needs and connect them with appropriate community resources Help patients navigate healthcare systems, insurance challenges, and social service programs Participate in team meetings and case discussions Utilize technology and documentation systems to coordinate patient care effectively Advocate for vulnerable and underserved patient populations

Master's Degree in Social Work (MSW) Required Minimum of 1 year of clinical experience in a medical, healthcare, or clinical setting Reliable transportation required Ability to travel to clinics, community meetings, and patient-related visits as needed Comfortable working independently while remaining highly collaborative with the team Strong computer and technology skills Excellent communication and relationship-building abilities

Bonus Points If You Have Bilingual skills (Spanish preferred) Experience working with Uninsured populations Underinsured populations Socially vulnerable patient populations Community outreach programs Knowledge of community resources, assistance programs, and healthcare navigation Experience coordinating care across multiple disciplines
