* Provide skilled nursing care and clinical assessments in a home setting
* Administer medications and treatments according to care plans
* Assist with complex Activities of Daily Living (ADLs) and mobility support
* Monitor health status, manage clinical paperwork, and provide medication reminders  
* Maintain clear, reliable communication with clients, families, and care coordinators 
✔️ What We Are Looking For:
* Active Massachusetts LPN License (Required)
* A compassionate, professional, and dependable work ethic
* Previous experience in home care, assisted living, or hospital environments is a plus
* Strong clinical communication and teamwork skills
* Reliable transportation to attend shifts on time
