CCBC is a non‑profit community mental health center serving over 50 communities in Massachusetts and growing. We provide inpatient, outpatient, and urgent care services to adults and children overcoming depression, anxiety, mood disorders, substance abuse, and many other conditions hindering personal fulfillment. Description of Position We are seeking a full‑time Care Coordinator to join our CBHC team in providing assistance to clients with mental health struggles. The ideal candidate is interested in providing a compassionate and full continuum of care to clients under the umbrella of the CBHC. You will be part of a multidisciplinary team that addresses the social, emotional, psychiatric, and medical needs of the clients we serve; this includes clients across the lifespan with a variety of presentations, including mental health and substance use concerns. If you are caring, dedicated, and ready to grow your career, we would love to hear from you! Responsibilities Coordinate medical and behavioral health care and make referrals. Identify, mitigate, and lift social and health inequities and barriers and advocate for the individual's access to services. Maintain regular contact with clients, providers, families, social service providers, and natural community supports to ensure treatment plans are implemented. Provide education and develop safety plans with clients and families. Qualifications Hold a Bachelor’s degree in a mental health field. Have at least three (3) years of human services related experience, preferably with individuals across the lifespan with serious mental illness and substance abuse. Possess good organizational skills and effective written, verbal, and computer skills. Have an ability to have a flexible work schedule. Receive ongoing training, professional development, and CEUs. Receive supervision and support when needed. Receive training in evidence‑based practices and support groups. Have opportunities for growth within the organization.
