Our home‑health registered nurse case managers lead and collaborate to deliver patient care plans within a team‑based environment. Responsibilities Lead and collaborate to manage and deliver patient care plans. Maintain accurate and timely charting of patient care. Support patients in Brownstown, Allen Park, south to the MI/OH Border and surrounding areas. Participate in a 24/7/365 after‑hours care team as needed. Utilize tablets, smartphones, and EMR software for documentation and communication. Requirements Registered Nurse (RN) license in the state where you will work. Graduate from an approved school of practical nursing. At least one year of RN experience and clinical competence in patient care. Valid driver’s license and ability to travel up to 20% of the day to patient locations. Commitment to meet charting deadlines consistently. Proficient in self‑management of time and scheduling.
