Physical Therapist - Per Diem

$150 per hour

Job Description

VNA of Ohio is one of Northeast Ohio's most established home health agencies - 124 years strong, CHAP-accredited, and 4-star rated by CMS. We're building our PRN PT team and offering top-quartile per-visit pay with a flexible schedule .

Pay Rates – Per Completed Visit

SOC / ROC - $150

Eval (complex skill add-on) - $115

Revisit / Discharge - $70

Downtime (pre-approved wait time) - $45/hr

Premium add-ons:

Same-Day / Urgent: +$25
Weekend: +$25
Holiday: +$50

"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.

Requirements

Must-Have:

Active PT license (Ohio) + current BLS certification
Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
Minimum 1 year of clinical physical therapy experience

Preferred / Nice-to-Have:

OASIS certification or demonstrated OASIS experience
Prior home health experience
Familiarity with HCHB or similar home health EMR
