Pediatric APRN

 

POSITION SUMMARY:  

The Pediatric APRN is responsible for managing health concerns and coordinating healthcare for individuals served at The Children’s Center of Hamden, in compliance with applicable licensing requirements and The Joint Commission standards, and within the scope of practice defined by the State of Connecticut. The Pediatric APRN collaborates closely with the treatment team, including the Director of Nursing, nursing staff, clinicians, and psychiatrists, and maintains regular communication with public and private agencies, as well as other medical providers, to ensure comprehensive and exceptional patient care.

The Pediatric APRN must have knowledge of the Standards of Care and Standards of Practice for Professional Nursing, as well as the State of Connecticut Nurse Practice Act. They should be knowledgeable in pediatric and adolescent growth and development, wellness care, medications, and immunizations, and have the ability to understand and apply licensing and accreditation standards and regulations. The APRN should be familiar with Continuous Quality Improvement (CQI) and Performance Improvement (PI) methods, as well as the oversight of Health Information Standards. They must be able to participate in Risk Management programming, incident trend analysis, and Root Cause Analysis with the development of corrective action plans, and contribute to strategic planning processes. The APRN should be capable of interpreting clinical outcome instruments and summary trend analyses, collaborating on policy and procedure development using best practice resources, and planning, organizing, and evaluating departmental work plans and program evaluations. Additionally, they must have knowledge of Health Information Confidentiality Laws and Statutes, and possess the skill to train and monitor Infection Control practices.

Position Status: Exempt

Accountable and responsible for the following results:

• Functions independently to perform comprehensive, age-appropriate medical histories, physical evaluations, medication consultations, and routine physicals for individuals served by the programs.
• Assesses and analyzes symptoms of illness or injury, formulates diagnoses, and determines appropriate interventions based on clinical evaluation.
• Orders medical consultations with specialists as indicated, follows up on recommendations, and ensures that treatment plans account for the growth, development, and specific needs of each individual. Identifies changes in signs and symptoms promptly and intervenes in emergent or acute situations.
• Orders, interprets, and evaluates diagnostic and therapeutic tests appropriate to the individual’s age, condition, and clinical needs.
• Prescribes and manages both pharmacologic and non-pharmacologic treatment modalities, monitoring efficacy and adjusting interventions as needed.
• Monitors the effectiveness of medical interventions and documents outcomes and follow-up care accurately and thoroughly.
• Facilitates transitions between healthcare settings, including providing necessary medication orders, coordinating follow-up care, and ensuring continuity of treatment post-hospitalization.
• Collaborates closely with multidisciplinary team members—including nursing staff, clinicians, and psychiatrists—to integrate medical care with other treatment interventions.
• Provides education to staff, individuals, and families to support informed decision-making and adherence to treatment plans.
• Identifies, reports, and develops plans to mitigate safety concerns in collaboration with the treatment team to maintain a safe environment for clients and staff.
• Delivers culturally competent medical care, demonstrating sensitivity to the diverse backgrounds and needs of all individuals served.
• Maintains accurate and up-to-date records, including medical files, DCF, DDS, DPH reports, and any other required documentation, ensuring compliance with regulatory and organizational standards.
• Remains knowledgeable of Policy and Procedure Manuals and provides expert consultation in the development, review, and implementation of policies and procedures related to medical care and clinical practices.
• Regularly review and renew all necessary certifications while completing mandatory training to remain in compliance with organizational and regulatory guidelines.
• Maintains certification in CCG training and safely assesses physical restraint techniques when required, ensuring the safety of clients and staff. 
• Maintains appropriate personal boundaries with young people and families served at The Children’s Center of Hamden, in accordance with the agency’s Boundary Code of Ethics.
• Maintains awareness of The Children’s Center of Hamden’s Policy and Procedure Manuals and accesses relevant information as needed to guide decision-making and ensure compliance.
• Contributes to a safe, welcoming, and equitable environment that reflects the agency’s commitment to diversity and inclusion.
• Perform other related duties as assigned.

MINIMUM QUALIFICATIONS

• Minimum of 2–5 years of post-graduate experience in pediatric primary care.
• Demonstrated ability to practice independently within the scope of licensure and professional standards.

LICENSURE /

CERTIFICATIONS

• Holds and maintains a valid Advanced Practice Registered Nurse (APRN) license in the State of Connecticut.
• Holds and maintains all required certifications, including Controlled Substance Registration, ensuring compliance with state regulations and professional standards.

• Ability to obtain and maintain current Basic Life Support and First Aid certification.

• Ability to obtain and maintain current Crisis Prevention and Intervention certification.
