Supervisor Care Management RN - Administrative Services

Job Title

Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team.

Job Description

Supervises Care Management (CM) staff to include nursing, social work, patient care coordinators and other non-clinical staff to achieve the goals for the organization; collaborates directly with inpatient and ambulatory clinicians and care teams, as well SMG and PCN leaders to support cost-effective high-quality medical services.

Required Qualifications

• Washington Registered Nurse License upon hire
• 5 years clinical nursing, including acute care hospital and/or ambulatory clinic experience.

Preferred Qualifications

• Bachelor's Degree
• 3 years Case Management experience including care management, care coordination, discharge planning, or population health.
• 2 years progressive supervisory experience.
