Community Care Liaison - Hospice

The Care Team Hospice is seeking a driven, relationship-focused Community Care Liaison (CCL) to grow our presence in the community. This is a high-impact role for someone who thrives on building connections, developing referral partnerships, and making a difference in the lives of patients and families.As a Community Care Liaison, you will be the face of our hospice services in the community. You’ll build and strengthen relationships with physicians, hospitals, and other referral sources to increase awareness, trust, and utilization of our services.Your success will be measured by your ability to grow census through strong partnerships, consistent outreach, and a deep understanding of hospice care.
