TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Buffalo, New York, 14221!

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience depending on the specialty. License: Must have active nursing license in hand. Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase (HCHB) and Oasis, with recent experience within the last 6 months. Description: Provides coordinated and directed home nursing care according to physician's orders. Utilizes agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Requires reliable transportation and a current driver's license. Must have agency-required liability insurance. Home visits will be conducted regularly. Periodic office meetings may be required. Will be assigned between 5-10 patients per day. Expected to complete approximately 30 points of productivity per week. May include tasks such as Start of Care, Wound Care, and Follow-up Visits. Must be comfortable with travel distances of up to 75 miles for patient visits. Mileage will be reimbursed at the standard IRS rate. All documentation must be completed within 24 hours of patient appointments. Delayed or incomplete documentation may result in patient care disruptions. No guaranteed hours are provided with this contract. Clinician must accept all scheduled patients regardless of distance, as declining may affect scheduling. Weekly productivity points must be met to ensure full-time status.
