Job Summary Occupational Therapist (OT) – Home Health, Per Diem position at Bella Terra Home Health and Hospice in the Inland Empire area. Compensation $50.00 - $60.00 per hour. Flexible scheduling. Key Responsibilities Implement standards of care for occupational therapy services in accordance with the home health plan of care. Provide patient, caregiver, and family education and counseling. Communicate effectively with physicians and healthcare team members. Evaluate patient’s functional status (muscle function, endurance, coordination, communication skills, self‑care ability, etc.). Assess home environment for safety and recommend adaptive equipment. Develop individualized treatment programs and set functional goals. Train patients in daily living activities, prosthetic/orthotic device use, and adaptive equipment. Fabricate splints as needed. Qualifications Valid California OT License. Current CPR certification. Minimum 1+ years of clinical experience as an Occupational Therapist. Reliable transportation and valid driver’s license with insurance.
