Job Title: Manager of Care Coordination (MCC) - Registered Nurse (RN) Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 12 hours (07:00 - 19:30) Experience: 1+ years of Case Management experience as a Registered Nurse (RN); 1+ years of nurse leadership experience License: Registered Nurse (RN) License & Registration Certifications: Basic Life Support (BLS) Must-Have: Bachelors of Science in Nursing (BSN) degree Passion for Care Coordination Description: Are you driven, dedicated, and passionate about caring for patients? Responsibilities: Collaborate and coordinate with multidisciplinary teams to manage safe and quality patient progression from admission to discharge. Act as a liaison between all patient care services to ensure appropriate bed assignment and optimal patient experience. Lead unit meetings to identify and eliminate barriers to patient care plans. Participate in escalation huddles, bringing attention to any barriers to discharge to key stakeholders. Serve as a clinical and technical resource and role model for staff, applying knowledge to daily practice.
