Transitional Housing Case Manager — 35-Hour Week

Breaking Ground is seeking a dedicated Case Manager for Transitional Housing in New York City. The Case Manager will provide essential services to help individuals experiencing homelessness transition to permanent housing. This role requires a Bachelor’s Degree and two years of relevant experience. Candidates should possess strong communication skills, knowledge of harm reduction techniques, and experience working with homeless individuals, particularly concerning substance use issues.
